SERVICES
Hospice provides a range of services to support patients and their families as they approach the end of life.
We provide comfortable care wherever our patients live by using a holistic approach toward their illnesses.
We also recognize that tending to the family’s emotional needs is as crucial as the patient’s needs.
Pain and symptom management
Hospice teams work to manage pain and other symptoms to ensure that patients are as comfortable as possible.
Emotional and
spiritual support
Hospice provides emotional and spiritual support to both patients and their families, including counseling, bereavement support, and access to chaplains or other spiritual leaders.
Care
coordination
Hospice teams work closely with patients' doctors and other healthcare providers to ensure that they receive coordinated and effective care.
Assistance with activities of daily living
Hospice teams can assist patients with activities of daily living, such as bathing, dressing, and eating.
Medical
equipment
and supplies
Hospice provides medical equipment and supplies to support patients' care, such as hospital beds, oxygen equipment, and wound care supplies.
Volunteer
services
Hospice volunteers can provide a range of services, including companionship, respite care, and help with errands.
PERSONALIZED
HOSPICE
CARE PLANS
Each patient receives a personalized care plan based on their specific goals, needs and desires in our hospice.
Our hospice clinicians will ask what really matters to the patient and family, and those items will be included in their individual plan of care.
We also support families and caregivers, so they can give their aging parent or loved one the best quality of life possible as they near the end of their life.
Care plans may include medical care, pain and symptom management, and emotional and spiritual support.
